Deep phenotyping - Imaging and functional testing in clinic, remote assessment and wearables

What?

Deep phenotyping and remote assessment using wearable devices and smartphones will be employed to identify subclinical damage or dysfunction in people experiencing long COVID.

Why?

We hypothesize that long COVIDÌýwill be associated with evidence of multi-organ pathology

Ìý

How?

This investigation will involve 300 to 350 participants (long COVID and controls) who will be recruited from existing population cohorts. The following investigations will be carried out:
Ìý

Multi-organÌýimagingÌý

Imaging of the brain, lungs, heart,ÌýliverÌýand kidneys usingÌýmagnetic resonanceÌýimaging (MRI)

Functional testing in clinic Ìý

  • ÌýAnthropometryÌý
  • ÌýLung function usingÌýspirometryÌýÌý

  • RestingÌýheart rate variability and electrocardiogram (ECG),Ìýlying, seated, standing brachialÌýand centralÌýblood pressure (BP)Ìý

  • Cardiopulmonary exercise testing, including assessment of peak VO2, 12 lead ECG, blood pressure, muscle perfusion using near infraredÌýspectroscopy, echocardiographic cardiacÌýoutputÌýand pre/post-exercise lactateÌý

  • ÌýRetinal Optical Coherence Tomography angiographyÌýÌý

  • ÌýStrength and physical performanceÌý

  • ÌýBloodÌýand urineÌýsamplesÌýfor storageÌý

Ìý

Remote assessmentÌýand wearables

  • ÌýMassSciÌýAppÌýfor remote assessmentÌýofÌýphysical and mental health fortnightly or monthly, depending on the parameter measured.Ìý

  • CognitronÌý()ÌýforÌýcognitive function, completed weekly for a month, and subsequently repeated 6 monthly.Ìý

  • GarminÌývivoactiveÌý4 wrist worn deviceÌýtoÌýcaptureÌýhabitualÌýactivity, heart rate variability, sleep patterns, oxygen saturation (SpO2) and respiratory rate. In addition,Ìýparticipants willÌýperform a standardised 6-minute walk test and a chair sit-stand test fortnightly for theÌýfirstÌý6 months, then quarterly. AllÌýremoteÌýassessments will continue for up to 18 months.Ìý
